Policy

Please find below our new patient medical forms. Filling them out prior to your visit will help both of us save time and see you sooner!

Standard Office Visits

  • Please arrive at least 10 minutes prior to your appointment time to allow for the time it takes for check-in procedures (15 minutes if you are a new patient since you will need to fill out forms.)
  • Kindly provide us with notice at least 24 hours prior to any cancellation or change of any appointment. Patients will be charged for no-show appointments or appointments that are cancelled with less than 24 hours notice.
  • Please bring all of your insurance information and appropriate identification to each office visit.
  • A parent or legal guardian must accompany children under 18 years of age.
  • For every visit, please bring a current list of all your prescriptions, over-the-counter medications, supplements and remedies.
  • Please allow 24 hours for prescription refills to be processed during weekdays.

Emergencies

  • If you ever have an emergency, please make the situation clear to our staff and we will immediately try to accommodate you.

Over the Phone Diagnosis and Treatment

  • Due to ethical and medical requirements, diagnosis and treatment plans can not be appropriately made over the telephone, via e-mail or facsimile. Accordingly, we are not permitted to simply “call-in” prescriptions for you unless you have been seen by a member of our professional staff.

Inclement weather

  • If the weather conditions are or may be hazardous, please contact our office to verify the status of your appointment.

Cancellation Policy

Appointment Confirmation Policy

  • We will be providing patient appointments booked online, which will require confirmation by our receptionist. Please provide a contact number online that you use or will check regularly. If we cannot confirm an appointment within 48 hours of the appointment, we reserve the right to cancel your appointment.
